Clopidogrel + Omeprazole: Are you still seeing this combination prescribed?
We had a patient come into the pharmacy today with a prescription for Clopidogrel 75mg alongside Omeprazole 20mg.
As most of us learned, Omeprazole inhibits CYP2C19, which reduces Clopidogrel’s activation and potentially increases stent thrombosis risk. However, many prescribers still stick to Omeprazole due to habit or cost, arguing that clinical outcomes aren't as severely impacted as trial data suggests.
When you run into this in your hospital or pharmacy, do you usually intervene and suggest switching to Pantoprazole or Rabeprazole, or do you just separate the dosing times? How do the doctors in your area usually respond to this intervention?
