Second-Line & Adjunctive Therapy
For Low-Grade Dysplasia: Endoscopic Eradication Therapy (EET), most commonly Radiofrequency Ablation (RFA), or intensive endoscopic surveillance every 6 months. For High-Grade Dysplasia: Endoscopic Mucosal Resection (EMR) for visible nodular lesions followed by RFA of the remaining flat Barrett segment. PPI therapy is often escalated to twice daily.
Surgical & Procedural Management

Preventive Strategies
Aggressive management of GERD symptoms. Screening EGD is recommended for men with chronic GERD symptoms (>5 years) and two or more risk factors (age > 50, Caucasian, central obesity, current or past smoking, confirmed family history of Barrett's or esophageal adenocarcinoma).
For non-dysplastic Barrett's, the prognosis is excellent with routine surveillance. If high-grade dysplasia develops, successful eradication with endoscopic therapies is achieved in >80% of patients. Survival drastically drops if invasive adenocarcinoma develops.
