Behcet disease is a rare, chronic, multisystemic inflammatory disorder characterized by recurrent oral aphthous ulcers, genital ulcers, and ocular inflammation (uveitis). It is classified as a variable-vessel vasculitis capable of affecting veins and arteries of any size.
Detailed Overview
The pathophysiology involves immune-mediated occlusive vasculitis. It is historically known as Silk Road Disease due to its high prevalence along the ancient trading route from the Mediterranean to East Asia. Severe morbidity often results from ocular involvement (leading to blindness) and central nervous system or large-vessel manifestations.
Epidemiology & Demographics
Highest prevalence in Turkey (up to 400 per 100,000), Japan, and the Middle East. Typically presents in the 20s or 30s. Equal gender incidence, but males generally suffer a more severe disease course.
Etiological Mechanism
Idiopathic. Believed to be an autoimmune or autoinflammatory response triggered by an environmental agent (potentially viral or bacterial) in a genetically susceptible individual (strongly associated with HLA-B51).
Primary Causes
Unknown specific cause; interplay of genetics (HLA-B51) and environmental triggers (e.g., infectious antigens like Streptococcus species).
Hyperactivity of neutrophils and dysregulated T-cell responses lead to immune complex deposition and neutrophil infiltration in the walls of blood vessels. This results in endothelial dysfunction, necrotizing vasculitis, and microthrombosis in small, medium, and large vessels (both arteries and veins).
Diagnostic Criteria & Guidelines
International Study Group (ISG) Criteria require: Recurrent oral ulceration (≥3 times in one year) PLUS at least two of the following: recurrent genital ulceration, eye lesions (uveitis), skin lesions (erythema nodosum, pseudofolliculitis), or a positive pathergy test.
Mucocutaneous flares: Topical corticosteroids (e.g., Triamcinolone acetonide 0.1% paste) and oral Colchicine 0.6 mg to 1.2 mg/day. Severe ocular/systemic disease: High-dose systemic corticosteroids (Prednisone 1 mg/kg/day) plus Azathioprine (2-2.5 mg/kg/day).
Second-Line & Adjunctive Therapy
For refractory ocular, vascular, or neurological disease: Anti-TNF agents (e.g., Infliximab 5 mg/kg IV at 0, 2, 6 weeks then every 8 weeks, or Adalimumab). Cyclophosphamide is used for life-threatening vascular/pulmonary aneurysms.
Surgical & Procedural Management
Endovascular embolization or surgical repair of large aneurysms (high risk of graft failure and pseudoaneurysm at anastomotic sites due to pathergy phenomenon in vessels).
Patient Counseling & Advice
Educate on the critical importance of immediate ophthalmology evaluation if any visual changes occur. Remind patients that the disease is relapsing and remitting, but medications can prevent irreversible damage like blindness.
Follow-Up & Monitoring Schedule
Regular follow-up with rheumatology every 3-6 months. Mandatory baseline and regular (every 3-6 months) slit-lamp examinations by an ophthalmologist.
Preventive Strategies
No primary prevention exists. Flare prevention relies on adherence to long-term immunosuppressive regimens (e.g., Colchicine, Azathioprine).
Variable. Mucocutaneous symptoms often burn out with age. Young males with early ocular or vascular involvement have the poorest prognosis and highest mortality risk (mostly from ruptured pulmonary aneurysms or neuro-Behcet).
