Fibromyalgia is a chronic, non-inflammatory central pain syndrome characterized by widespread musculoskeletal pain, fatigue, sleep disturbances, and cognitive dysfunction. It arises from dysregulation in the central nervous system's processing of pain signals rather than tissue damage.
Detailed Overview
Patients with fibromyalgia experience a heightened state of pain sensitivity (central sensitization). They lack objective signs of inflammation or joint damage on physical exam or imaging, which often leads to misdiagnosis or skepticism. It frequently co-occurs with other central sensitivity syndromes like irritable bowel syndrome (IBS) and chronic headaches. Management requires a multidisciplinary approach focusing on exercise, cognitive behavioral therapy, and neuro-modulating medications.
Epidemiology & Demographics
Affects 2-8% of the global population. Highly female predominant (female-to-male ratio is roughly 2:1 to 3:1). Usually diagnosed between ages 20 and 55.
Etiological Mechanism
The exact cause is unknown. It is believed to be triggered by physical trauma, severe psychological stress, or viral infections in genetically susceptible individuals.
Primary Causes
Idiopathic central nervous system dysfunction. Triggers include emotional trauma, PTSD, MVCs, or illnesses like Epstein-Barr Virus.
- Female gender: Much more common in women.
- Rheumatic disease: Having RA, Lupus, or Ankylosing Spondylitis increases the risk of secondary fibromyalgia.
- Psychological stress/trauma: History of PTSD, depression, or abuse.
The core mechanism is "central sensitization" of the nervous system. There is an amplification of nociceptive (pain) signals and a deficit in descending inhibitory pain pathways (which normally utilize serotonin and norepinephrine to blunt pain). Functional MRI studies show hyperactivity in the somatosensory cortex and insula in response to non-painful stimuli (allodynia). Elevated levels of Substance P and glutamate in the cerebrospinal fluid enhance pain signal transmission. The lack of restorative slow-wave sleep prevents normal tissue recovery and exacerbates fatigue and pain.
Characteristic Clinical Presentation
- Widespread pain: Aching or burning pain present on both sides of the body, above and below the waist.
- Severe fatigue: Profound exhaustion not relieved by sleep.
- Fibro fog: Cognitive difficulties with short-term memory, concentration, and word-finding.
- Unrefreshing sleep: Waking up feeling tired, frequent nighttime awakenings.
Physical Examination Signs
- Tenderness on palpation in multiple anatomic regions (classic tender points, though no longer strictly required for diagnosis).
- Normal joint exam (no synovitis, no restricted range of motion).
- Normal neurological exam without motor deficits.
- Depression and Anxiety: Highly prevalent secondary to chronic pain and reduced quality of life.
- Social isolation: Due to inability to participate in social activities or work.
- Opioid dependence: Iatrogenic complication from inappropriate prescribing of opioids.
Diagnostic Criteria & Guidelines
ACR 2016 criteria: Widespread Pain Index (WPI) >= 7 and Symptom Severity (SS) scale >= 5, OR WPI 4-6 and SS >= 9. Symptoms must be present at a similar level for at least 3 months, without another disorder explaining the pain.

Laboratory Tests & Biomarkers
- ESR and CRP: Strictly normal. Used to rule out inflammatory arthritis or PMR.
- TSH: Normal. Used to rule out hypothyroidism.
- ANA and RF: Usually negative. Should only be ordered if specific clinical signs of lupus/RA are present.
Imaging Modalities & Findings
- X-rays: Normal. Imaging is only indicated to rule out structural pathology (e.g., osteoarthritis) in specific joints.

Manageable pain with preserved ability to work and perform ADLs.

Significant pain and fatigue requiring regular medication and activity modifications.

Debilitating pain and severe brain fog leading to disability and inability to work.
Non-pharmacologic: Graded aerobic exercise (the most effective treatment) and Cognitive Behavioral Therapy (CBT). Pharmacologic: SNRIs like Duloxetine (30-60 mg PO daily) or Milnacipran (50 mg PO BID) to boost descending inhibitory pathways, or Gabapentinoids like Pregabalin (75-150 mg PO BID) to reduce neurotransmitter release.
Second-Line & Adjunctive Therapy
Tricyclic antidepressants: Amitriptyline 10-25 mg PO QHS to improve sleep architecture and pain. Cyclobenzaprine 5-10 mg PO QHS for muscle spasm/sleep. (Note: NSAIDs, acetaminophen, and opioids are generally ineffective and not recommended).
Surgical & Procedural Management
Not applicable. Surgery often acutely exacerbates fibromyalgia symptoms.
Recommended Lifestyle Changes
- Low-impact aerobic exercise (swimming, walking, cycling) starting very slowly and building up.
- Strict sleep hygiene practices (dark room, consistent sleep schedule).
- Stress reduction techniques such as mindfulness meditation or yoga.
Patient Counseling & Advice
Validate the patient's illness; assure them the pain is real but not destructive to their joints or organs. Emphasize that passive treatments (pills, massages) are less effective than active treatments (exercise, CBT).
Follow-Up & Monitoring Schedule
Follow up every 3 months initially to titrate medications and assess adherence to exercise programs. Monitor for emergence of clinical depression.
Preventive Strategies
No proven primary prevention. Early intervention after physical/emotional trauma may reduce the risk of developing centralized pain.
Chronic and fluctuating course. It is not life-threatening and does not cause physical deformity. Most patients adapt and maintain function with multidisciplinary management, though true "cure" is rare.
