While most people experience stress reactions following trauma, PTSD is diagnosed when these symptoms persist beyond one month and cause significant functional impairment. The trauma can be a single event (e.g., car accident, assault, natural disaster) or complex, prolonged exposure (e.g., childhood abuse, combat). The condition fundamentally alters the brain's fear-processing network, causing the individual to perceive current safety through the lens of past trauma. Etiological Mechanism

  • Suicidality: Significant risk of suicidal ideation and completed suicide.
  • Cardiovascular Disease: Chronic high sympathetic tone increases long-term risk of hypertension and coronary artery disease.

Diagnostic Criteria & Guidelines

DSM-5 criteria require exposure to a traumatic event, followed by symptoms from 4 clusters lasting >1 month: 1) Intrusion (≥1 symptom), 2) Avoidance (≥1 symptom), 3) Negative alterations in cognitions and mood (≥2 symptoms), 4) Alterations in arousal and reactivity (≥2 symptoms). Symptoms must cause clinically significant distress or impairment.

Preventive Strategies

Providing rapid psychological first aid, ensuring safety, and fostering social support immediately after a trauma reduces PTSD risk. Routine debriefing (forcing people to recount the trauma immediately) is NOT recommended and may increase risk.

Highly variable. With evidence-based trauma-focused therapy, up to 50-60% of patients achieve remission. However, roughly one-third develop chronic, unremitting symptoms requiring lifelong management.
