Emergency Management: Scleroderma Renal Crisis (SRC). Characterized by sudden malignant hypertension, microangiopathic hemolytic anemia, and acute renal failure. Treatment is an absolute emergency: DO NOT USE BETA BLOCKERS. MUST use ACE Inhibitors immediately and aggressively (Captopril 12.5-25 mg PO every 8 hours, titrated rapidly to maximum dose) even if serum creatinine is rising, to reverse the efferent arteriolar vasoconstriction.
Systemic Sclerosis is a rare, complex, immune-mediated multisystem disease characterized by widespread vascular dysfunction, autoimmune activity, and progressive fibrosis of the skin and internal organs (notably the lungs, GI tract, heart, and kidneys).
Detailed Overview
SSc is one of the most severe rheumatologic diseases, with the highest case-fatality rate among systemic autoimmune disorders. The disease begins with microvascular injury, leading to tissue hypoxia, which drives an aberrant immune response and unchecked fibroblast activation. This results in massive deposition of collagen and extracellular matrix. It is broadly divided into Limited Cutaneous SSc (lcSSc, formerly CREST syndrome) and Diffuse Cutaneous SSc (dcSSc), which dictate the pattern of skin involvement and risk of internal organ complications.
Epidemiology & Demographics
Incidence is roughly 1-2 per 100,000 annually, with a prevalence of 15-25 per 100,000. It predominantly affects women (female-to-male ratio of 4:1 to 8:1) with a typical onset between 30 and 50 years of age. African Americans tend to develop more severe, diffuse disease at a younger age.
Etiological Mechanism
The exact trigger is unknown. A genetic predisposition exists (HLA-DRB1, STAT4, IRF5) combined with suspected environmental triggers such as silica dust exposure, organic solvents, and certain viral infections (CMV, Parvovirus B19).
Primary Causes
Idiopathic. A triad of 1) microvascular injury, 2) innate and adaptive immune dysregulation (autoantibodies), and 3) autonomous fibroblast activation resulting in pathologic fibrosis.
- Genetics: Family history of autoimmune diseases; specific HLA haplotypes increase susceptibility.
- Occupational Exposures: Long-term exposure to silica dust (coal miners, sandblasters), polyvinyl chloride, or organic solvents.
- Bleomycin: Chemotherapeutic agent known to induce scleroderma-like fibrotic changes.
The inciting event is endothelial cell injury in small vessels, causing dysregulation of vascular tone (Raynaud's) and release of endothelin-1 (a potent vasoconstrictor and profibrotic agent). Endothelial apoptosis leads to capillary loss and tissue hypoxia. Hypoxia and endothelial damage recruit macrophages and T-cells, which secrete profibrotic cytokines, principally Transforming Growth Factor-beta (TGF-beta) and Platelet-Derived Growth Factor (PDGF). These cytokines continually stimulate fibroblasts to differentiate into myofibroblasts. The myofibroblasts overproduce type I and type III collagen, leading to dense fibrosis of the skin and internal organs. The microvascular obliteration also causes pulmonary arterial hypertension and renal crisis.
Characteristic Clinical Presentation
- Raynaud's Phenomenon: Episodic, cold- or stress-induced vasospasm of the digits causing a triphasic color change: pallor (white), cyanosis (blue), and reactive hyperemia (red). Almost universally the first symptom.
- Skin Thickening: Skin feels tight, hard, and shiny, limiting joint mobility. Puffy hands early on.
- Dysphagia and Reflux: Difficulty swallowing and severe heartburn due to esophageal dysmotility and lower esophageal sphincter incompetence.
- Dyspnea: Shortness of breath due to interstitial lung disease or pulmonary hypertension.
Physical Examination Signs
- Sclerodactyly
- Microstomia
- Telangiectasias
- Digital Pitting Ulcers
- Interstitial Lung Disease (ILD): Pulmonary fibrosis; the leading cause of death in SSc (especially in dcSSc).
- Pulmonary Arterial Hypertension (PAH): Group 1 PH caused by remodeling of pulmonary arterioles; a major cause of mortality (especially in lcSSc).
- Scleroderma Renal Crisis: Abrupt onset of malignant hypertension and rapidly progressive acute kidney injury due to fibrointimal proliferation in renal arteries.
Diagnostic Criteria & Guidelines
Diagnosed based on the 2013 ACR/EULAR classification criteria. A score >= 9 is required. Skin thickening of the fingers of both hands extending proximal to the metacarpophalangeal joints is sufficient alone (score 9). Otherwise, points are tallied for sclerodactyly, digital ulcers, telangiectasia, abnormal nailfold capillaries, PAH/ILD, Raynaud's, and SSc-specific autoantibodies.
Differential Diagnosis
- Nephrogenic Systemic Fibrosis (post-gadolinium in renal failure)
- Eosinophilic Fasciitis (spares the hands/face)
- Morphea (localized scleroderma, no internal organ involvement)
- Primary Raynaud's Disease (no tissue necrosis or fibrosis)
Laboratory Tests & Biomarkers
- ANA: Positive in >90% of cases (usually nucleolar, centromere, or speckled pattern).
- Anti-Centromere Antibody: Highly specific for Limited Cutaneous SSc. Associated with lower risk of lung fibrosis but high risk of PAH.
- Anti-Scl-70 (Anti-topoisomerase I): Highly specific for Diffuse Cutaneous SSc. Strongly predicts interstitial lung disease.
- Anti-RNA Polymerase III: Strongly associated with rapidly progressive skin thickening and high risk of Scleroderma Renal Crisis.
Imaging Modalities & Findings
- Nailfold Capillaroscopy: Enlarged 'giant' capillaries, hemorrhages, and avascular areas (capillary dropout) are a hallmark of secondary Raynaud's in SSc.
- High-Resolution CT (HRCT) Chest: Ground-glass opacities (early active alveolitis) and reticular opacities/honeycombing (late fibrosis), predominantly at the lung bases.
- Barium Swallow: Dilated esophagus with absent primary peristalsis in the lower two-thirds (smooth muscle atrophy).

Limited Cutaneous (lcSSc)
Skin thickening restricted to areas distal to the elbows and knees, and the face. Historically known as CREST (Calcinosis, Raynaud's, Esophageal dysmotility, Sclerodactyly, Telangiectasia). High risk of late-onset Pulmonary Arterial Hypertension.

Diffuse Cutaneous (dcSSc)
Skin thickening extends to proximal extremities and the trunk. Rapid progression. High risk of early interstitial lung disease and Scleroderma Renal Crisis.
Organ-based symptomatic treatment, as no drug reverses established fibrosis. Raynaud's: Calcium Channel Blockers (Amlodipine 5-10 mg PO daily). GERD: High-dose Proton Pump Inhibitors (Omeprazole 40 mg PO BID). ILD: Mycophenolate mofetil 1-1.5g PO BID or Cyclophosphamide.
Second-Line & Adjunctive Therapy
For severe Raynaud's/digital ulcers: PDE5 inhibitors (Sildenafil) or IV Iloprost. For progressive ILD: Antifibrotics like Nintedanib 150 mg PO BID or Tocilizumab (IL-6 inhibitor). For PAH: Endothelin receptor antagonists (Bosentan, Macitentan).
Surgical & Procedural Management
Digital sympathectomy or amputation for non-healing, gangrenous digits. Lung transplantation for end-stage ILD or PAH. Autologous Stem Cell Transplantation (ASCT) is highly risky but may be considered for severe, rapidly progressive diffuse disease.
Recommended Lifestyle Changes
- Avoid cold exposure entirely. Wear multiple layers, heavy gloves, and use chemical hand warmers to prevent Raynaud's attacks.
- Elevate the head of the bed and avoid eating within 3 hours of bedtime to prevent severe acid reflux and microaspiration.
- Perform daily skin stretching and range-of-motion exercises to delay joint contractures.
Patient Counseling & Advice
Educate patients on the critical importance of home blood pressure monitoring. ANY sudden rise in blood pressure, especially if associated with headache, must be reported immediately as it may indicate impending Scleroderma Renal Crisis. Avoid corticosteroids (Prednisone > 15 mg/day) as they can precipitate renal crisis.
Follow-Up & Monitoring Schedule
Annual Echocardiogram and Pulmonary Function Tests (Spirometry and DLCO) to screen for PAH and ILD. Routine monitoring of blood pressure. Regular dermatologic exams for skin thickening (modified Rodnan skin score).
Preventive Strategies
No prevention for the disease itself. Prevention focuses on avoiding complications (e.g., using Amlodipine to prevent digital ulceration, keeping hands warm).
Highly variable. Limited SSc has a better survival rate but high morbidity from GERD and digital ulcers; 10-15% will develop deadly PAH late in the disease. Diffuse SSc has a worse prognosis, with 10-year survival around 70%, primarily due to early ILD or renal crisis.
