A Student’s Step-by-Step Guide to ECG Interpretation: Rhythm, Axis & Ischemia

Interpretation of the 12-lead electrocardiogram (ECG) is a mandatory skill for medical students, pharmacy scholars, nurses, and emergency practitioners. Approaching every ECG systematically prevents missing subtle pathological abnormalities such as ischemia, electrolyte imbalances, or conduction blocks.

The 5-Step Systematic Approach

  1. Rate & Rhythm: Calculate heart rate (300 / number of large boxes) and inspect P waves for sinus rhythm.
  2. Axis: Evaluate Leads I and aVF to determine quadrant axis direction.
  3. Intervals: Measure PR interval (120–200 ms), QRS duration (<120 ms), and QTc (<450 ms men, <470 ms women).
  4. Hypertrophy: Inspect V1 and V5/V6 for Left Ventricular Hypertrophy (Sokolow-Lyon criteria).
  5. Ischemia & Infarction: Search for ST elevation (STEMI), ST depression, or inverted T waves across anatomical lead groups.

Determining Cardiac Axis: The Two-Lead Method

The cardiac axis reflects the major direction of electrical depolarization through the ventricles:

  • Normal Axis (0° to +90°): Lead I is Positive (+) and Lead aVF is Positive (+).
  • Left Axis Deviation (-30° to -90°): Lead I is Positive (+) and Lead aVF is Negative (-). Common in LVH, LABB, or inferior MI.
  • Right Axis Deviation (+90° to +180°): Lead I is Negative (-) and Lead aVF is Positive (+). Associated with RVH, PE, or COPD.

Identifying Acute Myocardial Infarction (STEMI Patterns)

ST-segment elevation ≥1 mm in two anatomically contiguous leads indicates transmural myocardial ischemia requiring emergent revascularization:

  • Anterior MI: Leads V1–V4 (Left Anterior Descending artery).
  • Inferior MI: Leads II, III, aVF (Right Coronary Artery).
  • Lateral MI: Leads I, aVL, V5, V6 (Left Circumflex artery).
